With some diabetic patients, the hormone insulin may be absent, yet necessary for their survival. A lot of negative buzz in the U.S. hisses "socialized medicine," partly because of the delays and restrictions associated with government run health care in many countries.

"That's not what I found in Costa Rica, at all," said retiree Gene Warneke from his home near Grecia, a sky-top villa overlooking Costa Rica's lush Central Valley. He now walks with a prosthetic leg in the aftermath of an amputation brought on by a clotting disorder.

As an American living with a disability in Costa Rica, Warneke dealt with his health care and medical challenges both in the U.S. and in Costa Rica. And the story of how he came to see the inside of socialized medicine in Costa Rica and government run insurance plans is relevant for people interested in the quality of healthcare they might experience if choosing to retire in Costa Rica.

"Before I retired in Costa Rica, I had been living and working in southern California and carried the "Cadillac" of medical coverage -- an 80/20 plan through Blue Cross," Warneke said. If that were all there was to tell, this would be one up-beat anecdote about so-called socialized medicine and government-backed insurance plans in Costa Rica. "With the Costa Rica INS plan, I didn't have one complaint about access, long lines, wait times, non-availability," Warneke said.

But there is one more chapter. It's about the medical mystery that robbed Warneke of his leg. Two years after the shoulder injury had faded to an occasional twinge, he began feeling a numbness in his left foot: blood clots in the femoral artery were starving his limb of oxygen, and gangrene was imminent. The failed artery was bypassed and the dying foot amputated. But the wound wouldn't heal and further surgery found the reason.

"A major vein to the surgical site below my shin clotted closed," He said. "The skin and bone couldn't heal without blood flow, and another vein bypass was required. Another amputation had to be performed several inches above my knee. "Did the surgeons perform according to world class standards? A friend of Warneke's living in California had the American Embassy in Costa Rica send over their doctor to consult with and review the work of Warneke's surgeons. He found that they had done the best any surgeon could have done, whether in the U.S. or anywhere else.

By now you might be extending Warneke condolences on the loss of his leg -- and condolences on the loss of his retirement kitty, right? Not quite. "Costa Rica's public socialized medical system covered this emergency," Warneke said. "The premiums were $32 a month and there was no co-payment as long as I used the hospitals and doctors in the plan, a system identical to HMO and PPO plans in the U.S.," he said.

The ultimate test, however, is the quality of care. "I'd rate both my American experience and my two Costa Rican experiences as medically equal," Warneke said. "The difference is the price tag; my first Costa Rican plan -- the equivalent of Blue Cross of California -- cost me less than a third of the American bill. The Costa Rican government's provider plan cost me next to nothing," he said.

Oh, wait. There was one big difference. After a hernia repair in California, Warneke had to be treated for a drug-resistant bacterial infection that he acquired in the U.S. hospital. In the Costa Rican episode, he opted to spend about a $1,000 for an elective hernia repair ($6500 in SoCal) and suffered no bacterial infection.

"No ex-pat in Costa Rica needs to worry about 'being taken care of,'" Warneke said. "Here, medical compassion and expertise, whether preventive, elective, or unexpected, unwanted, or even a life-threatening emergency, is an integral part of this society."

Warneke has been featured in television coverage and in August 28 article in AIDia.cr. regarding the new prosthetic micro-processing unit in his leg. "It seems I'm the first in Central America to have this type," he said. No matter where you reside or where you're looking to settle down, if you're a first-time homebuyer, there's a program that can help you purchase a house. And virtually every type of residence is eligible under these programs, including single-family homes, condominiums, townhouses, modular homes, and manufactured housing. Many assistance programs have income limitations, particularly those that provide city, state or federal funding. But other programs have no income criteria. Also, certain housing assistance plans impose caps on the purchase price of the property you can buy. Despite these restrictions, you'll find that taking advantage of a first-time homebuyers' program is one of the smartest things you can do. With this program, which is through the State of New York Mortgage Association, you also receive a minimum of $5000, or 5% of your mortgage amount, whichever is higher, in order to pay your down payment and closings costs.

In Texas, you can get grant funds up to 5% of your mortgage amount, along with two type of loans with interest rates that are typically 1% below current market rates via the Texas First Time Homebuyers Program. In Illinois, first-time homebuyers taking place in the Assets Illinois Homeownership Project can receive a dollar for dollar match up to $2,000 to help them save for the purchase of a first home. In South Carolina, the Single Parent Program is open not just to first-time homebuyers, but to any one renting, as long as the person has a child under the age of 18 and the homebuyer is divorced or has been separated for six months. In California, the High Cost Area Home Purchase Assistance Program (HiCAP) offers up to $7,500 in down payment assistance in the form of a deferred-payment second loan. For more information, call 877-922-5432.

In Georgia, the Dream Homeownership Program offers 100% financing via a low interest rates 30 or 35-year mortgage, and a second loan ranging from $5,000 to $20,000 that can be used for a down payment and closing costs. The down payment assistance loan has no interest, no monthly payments, and no payment is due until the house is sold, refinanced or no longer used as the buyer's primary residence. In Pennsylvania, the HOMEstead Down payment and Closing Cost Assistance Loan features up to $20,000 in down payment and closing cost assistance in the form of a no-interest second loan. There are many times when we need to evaluate the services we provide; whether we are working in public sector providing front-line services to the community, or freelancing developing websites for business and personal clients. Evaluation helps us provide more effective and efficient services. This enhances the quality of our services, or increases our profits.

As the aim of evaluation is to become more effective and efficient, when we evaluate a service we need to think about two distinct areas; the processes involved in providing the service, and the effect or impact of the service. When we speak of evaluating processes what do we mean? Basically we are talking about a thorough examination of the ways in which a service is delivered. How is the service managed? How well do different people involved in delivering the service communicate with each other? How is service delivery project managed? Where are the roadblocks to effective and efficient delivery of the service? What do we do that we do not need to do? What do we not do that we ought to do?

The answers to these questions vary enormously. The management of a public sector service is very different to the management of a small design agency. Before we can answer any of these questions we need to know what we actually do. Evaluation is not complicated, but it is essential. There is no benefit to anyone in providing a costly service that is relatively ineffective. Monitoring and consultation provide practical evidence on which to make rational judgments on the effectiveness and efficiency of the service we deliver. In other words, monitoring and consultation provide us with the tools for evaluation. This in turn helps us logically become more effective and efficient in delivering our service.

HIPAA Provides Medical Information Protection by Federal Law Mandate - Your Rights and Remedies

The Health Insurance Portability and Accountability Act (HIPAA) provides rights and protections for participants and beneficiaries of group health plans. The Privacy Rule, a federal law, grants health consumers rights over their information and promulgates rules and limitations on who can look at and receive personal health information. The Privacy Rule applies to all forms of individuals' protected health information, whether electronic, written, or oral.

The Security Rule, a Federal law that protects electronic health information, requires HIPAA-covered entities to ensure that electronic protected health information is secure.

Additionally, HIPAA includes protections limiting exclusions for preexisting conditions; prohibits discrimination against employees and dependents based on health status; and allows an opportunity to enroll in a new plan to individuals in certain circumstances. HIPAA may also grant a right to purchase individual coverage if no group health plan coverage is available, and for those who may have exhausted COBRA or other continuation coverage.

The following are some of your rights under HIPAA:

You have the right to receive a copy of your health records
You can ask to see and receive a copy of your medical records and other related health information. In most cases, copies must be provided within 30 days of being requested, though there may be a fee associated with the cost of copying and mailing.
You can ask to have corrections added to your health information
You can ask that any misinformation in your file be corrected, or you may request to add information to an incomplete file. In most cases the file should be changed within 60 days of the request to amend or change.

In most cases, you should receive this notice on your first visit to a provider, or in the mail from your health insurer. Additionally, you may request a copy at any time.
You can decide whether to give your permission before your information can be used or shared

Generally speaking, your health information cannot be given to your employer, used or shared for things like sales calls or advertising, or used or shared for many other purposes unless you grant express permission by signing an authorization form. This authorization form must tell you who will get your information and what the information will be used for.

Who can receive and view your health information?

To ensure your health information is protected in a way that does not interfere with treatment, your information can be used and shared:
For treatment and care coordination;
To compensate doctors and hospitals for your health care and help run their businesses;
With your family, relatives, friends, or others you identify who are involved with your health care or payment, unless you object;
To ensure doctors give good care and nursing homes are clean and safe;

Your health information cannot be used or shared without your written permission unless this law allows it. For example, without your authorization, your provider generally cannot:
Give your information to your employer;
Use or share your information for marketing or advertising purposes; or
Share private notes about your health care.
You may request that your information not be shared
You can ask your provider or health insurer not to share your health information with certain people, groups, or companies, such as with other doctors or nurses in a particular hospital or clinic. However, they do not have to agree to do what you ask.
You have the right to file a complaint
You may file a complaint with your provider or health insurer if you suspect your information was used or shared in a way that is disallowed under the privacy law, or if you feel you were unable to exercise your rights.

Who must follow this law?

Most doctors, nurses, pharmacies, hospitals, clinics, nursing homes, and other health care providers. Also required to follow this law are health insurance companies, HMOs, most employer group health plans, and certain government programs such as Medicare and Medicaid.

Who is not required to follow this law?

Life insurers;

Workers compensation carriers;

Most schools and school districts;

Many state agencies, for example child protective services;

Most law enforcement agencies; and

Many municipal offices.

When you make an original visit to a doctor's office, hospital, or other health care provider, you should be given a copy of your HIPAA rights, which you will be asked to sign. Make sure you read the entire document, and request a copy for your own files.
