Wanted - Healthcare Workers!

There are critical shortages of these men and women all over the United States in medical practices and hospitals everywhere. Therefore, healthcare options provide one of the fastest growing professional fields for Baby Boomers and Active Seniors. Let's look at healthcare careers that you can pursue right now.

Healthcare administration worker

There are unlimited possibilities because the hospitals nationally are all adding "new wings," particularly for cancer and cancer research, heart surgery and other specialties, which different hospitals pursue.

Information Technology knowledge will increasingly be required in most of these positions. It's no longer someone laboriously keeping records by hand anymore, but it's an individual entering the information into a computer at a reception desk, for example. Today's medical community is increasingly moving toward a paperless trail of information. Companies such as Kansas City's Cerner, which is a New York Stock Exchange company where my son Jim works, are very much on the cutting edge of this type of development.

Nurse practitioner

This is going to be a growing field in the future. Walgreens and one of the other major pharmaceutical chains have now established urgent care facilities owned by their respective firms, which feature a nurse practitioner for more routine procedures.

So the overall category of nurses in our society, especially as Baby Boomers continue to age, will exceed a one million nurse shortfall in our society in the future, which makes this a really critical issue in our society. Fortunately, there are men and women who have been previously trained as nurses, especially women who may have left the profession to raise a family, for example. The nursing profession today is reaching out to these women to encourage them to come back to their nursing career later in life.

Nurse educator

In order to address the increased demand for nurses, there is also today a crying need for nurse educators, who are willing to go back to school to earn a Master's degree or PhD so that they can be qualified to teach in a nursing school or at a college level.

Medical technologist and hospital worker

These men and women do all sorts of testing, CT scans and various things having to do with specialized practices in medicine or as workers in a hospital.

Dental hygienist

This professional also receives high pay in a dental office. Typically, this individual is a woman, who works as an independent contractor for a dental office or several dental practices.

This high demand individual has typically pursued a two year curriculum at a local junior college. But, the sky is the limit in terms of where that woman will wind up working. Most often, she will be able to choose a dental office or offices close to her home. Each morning, Lois Vernon, who is 96 years old, starts her day at the lodge of Ashworth with a little assistance from her home care companion, Kathy Anderson.

For Vernon, who has difficulty seeing, having Anderson come to her apartment for two hours to fix her breakfast and assist her with getting dressed and getting down: to the exercise room is "a big help," she said. "She's wonderful," Vernon said of Anderson, who is employed by Comfort Keepers, one of three national home-care franchises that operate in Greater Des Moines. As the number of seniors continues to grow across the United States, the demand for non-medical services that can help people stay in their homes longer will multiply, experts say. More than 33 million U.S. residents, or 13 percent of the population, are already over age 65, and their ranks are expected to double by 2030, when an estimated one in five Americans will be over 65. The need for these services, which range from light housekeeping, cooking and running errands to persona! Services such as baths are particularly great in Iowa, which in 2000 had the fourth-highest percentage of over-65 residents in the country.

Nursing homes routinely are unable to staff enough nursing assistants and this shortage leads to a high turnover and burnout, and this leads to poor care.

Nursing assistant staffing sometimes inexplicably decreases. What used to be a required five nursing assistants on the floor suddenly becomes four and temporary or agency nursing assistants, although usually competent, are often hired to save money. But it is difficult to provide quality care when someone is not familiar with the residents. Even if inadequate staffing is cited, fines and citations are often reduced and even eliminated through loopholes in the laws and extended, multiple appeal procedures.

Visit your loved one in the evening and occasionally on different nights when the staff does not expect you. This time period after the 9-5 staff has left and before residents are assisted to bed is usually the time of day when the quality of care is at its worse. (Melcher 1988)

Malnutrition is more prevalent in nursing homes. Providing enough time and the proper technique and attitude to getting a resident fed is extremely important. If your loved ones cannot communicate, then it's better for you to post a clearly written sign in her room describing the best way to get her to eat.

Making an official complaint with the nursing home division of your state's department of public health may be a necessary option.

The nurse gives an example of complaint, which will be followed by lawsuit:

"I have personally been involved in a negligence case. A couple of years ago eight nurses, including myself, received certified letters from the MA Board of Nursing, "You neglected Mrs. _____ at the So & So Nursing Home and she got a bedsore. How do you respond?" That was the gist of it. This event happened 6 months prior to receiving this letter (I had worked 2 shifts that week). All eight nurses had also been questioned by DPH investigators a couple of months prior to this formal complaint - this resident, a mostly bed bound woman in her late 80's with a variety of serious medical conditions, died several months after this bedsore was discovered, of other causes (treatment did heal this wound). Her daughter, who I believed, and others believed also, had unrealistic expectations of day-to-day care, filed a complaint and eventually there was a lawsuit. Every nurse who had worked a shift during the week prior to the discovery of the bedsore was accused and had to submit substantial documentation about their entire nursing career

Agency or temporary nurses (and nursing assistants) are used by many long-term care facilities and hospitals. Despite paying a higher hourly rate, the facility actually saves money by offering no benefits. The agency, hospital, or nursing home administrator will tell you all temporary staff is thoroughly trained, competent, and provided orientation to the facility, but sometimes the reality is quite different, because temporary placement services is a very competitive and cutthroat business (a nursing home or hospital often calls several agencies, whoever calls back first with a nurse who can work the shift needed, gets the contract), the agency's goal is to get a "body" to the facility. As long as they don't receive a serious complaint, they're satisfied. (Melcher 1988)

Racism continues to be a part of the fabric and tradition of American society and has adversely affected the health care system in general, and the profession of nursing in particular. 90% of Director's of Nursing (DON) in nursing homes are white females while the majority of nursing assistants are individuals of color. Too often racism is manifested in the attitudes of DON's and nurse managers towards nursing assistants. This destructive process has a tremendous negative effect on any attempt to improve quality of care.

Medical Coding Jobs From Home

Working from home and being paid well for it is a goal of many individuals. While searching out ways to accomplish this goal many have stumbled upon a niche field in medical administration called "medical coding".

Medical coding positions are present in hospitals, physician offices, insurance companies, home health agencies, and just about anywhere medical services are being provided and billed for. In addition to traditional hourly employment in this field there are also a number of opportunities to do this type of work from the comfort of your own home.

A standard, hourly, medical coding job is what is usually advertised in the classifieds and help wanted ads. These positions usually require individuals to be present at a specific location, punch a time clock, ect. Employees are usually paid at an hourly rate and receive benefits such as paid vacation, health insurance, and paid holidays.

In contrast, most work from home medical coding jobs are contract positions. This means that a larger company who has multiple clients farm out the work into smaller portions on an individual basis. Individual medical coders who are awarded a portion of this work are called independent contractors and are considered self employed.

Most often an independent contractor will agree to receive a specified amount of work each week, code it within a specific time frame, maintain a certain accuracy standard, and in return are usually paid on a "per. chart coded" basis. While such positions usually do not include benefits such as paid vacation, medical insurance, and paid holidays it does offer an extremely flexible schedule and the ability to work from home, which saves on travel, attire, and other related expenses.

An independent contractor can expect companies that award coding contracts to provide the work and coding software while they will be responsible for providing the computer hardware, internet connection, and medical coding books. Independent contractors are also responsible for withholding their own taxes since their payments are made on their gross earnings. At year end individuals will receive a 10-99 instead of a W-2 and will need to file schedule 1040. This can also be a benefit since work related expenses such as coding books, internet, computer, ect. can be considered a tax deduction.

Most independent contraction positions are available for a specific specialty, such as pediatrics, emergency medicine, or dermatology. These positions normally require that contractors hold either a CPC or CCS coding credential and have a minimum of three years of coding within their specialty.

Each coding specialty has its own bar rate and standard pay rate. These are important to know when discussing a contract. For example, when coding emergency medicine the standard bar rate 25 charts an hour and the average pay rate is between.65 and.75 cents per. chart for professional coding. Based on these numbers and an average work week of 40 hours (for full time) or 20 hours (for part time) an independent contractor can choose the number of charts to contract for each week based on their own speed.

For example, individuals who code the standard 25 charts an hour and only wish to work 4 hours per day (that's a 20 hour week) should contract for 500 charts per week and could expect to gross between $352 and $375 each week. The faster an individual can code the more money they can make, so an individual who can code 50 charts per hour can either choose to contract for full time work and receive full time earnings while working only part time hours, or they can choose to contract for a large number of charts and increase their earnings.

While speed is important to an independent contractor's earning potential accuracy is equally important. Most companies who award coding contracts stipulate that the medical coder must maintain a certain accuracy in their coding in order to keep their contract. The accuracy rate usually falls between 96% - 99%. Charts coded by the medical coder are routinely audited by the company to ensure their rates are being maintained.

Prior to being considered for a work from home medical coding job there are several things one must do. The first is to earn either a CPC or CCS coding credential. While there are multiple coding credentials that are awarded by numerous organizations these are the only two that are commonly recognized among employers nationwide.

The CPC stands for Certified Professional Coder, and is awarded by the American Academy of Professional Coders (AAPC). The second credential, the CCS, stands for Certified Coding Specialist and it is awarded by the American Health Information Management Association (AHIMA).

Each of the two credentials can be earned by sitting for and passing the 5 - 6 hour coding exam. Proper education prior to sitting for either exam is strongly encouraged and selecting the right organization to prepare you should be done carefully.

Once an individual has earned either their CPC or CCS credential the next achievement should be to gain some on the job coding experience. This is required by most companies that award independent coding contracts. It is also more beneficial for a medical coder to start out this way as well.

Succeeding as an independent contractor is dependent upon both speed and accuracy and a new coder will be neither. It is best to earn an hourly paycheck when starting out when you can code as slow as you need to and you are able to ask as many questions as you may have to.

Once an individual has gained two to three years of on the job medical coding experience they will be considered for most independent coding contract positions. The next step is to find such companies that offer these types of positions or contracts.

Most internet searches for "at home medical coding jobs" and "medical coding jobs from home" usually return more solicitation or articles than job leads. It is better to use search phrases such as "medical coding independent contractor" or "remote medical coding jobs". Attending local AAPC chapter meetings is also a good way to hear of such opportunities from the local coding community. In addition, an individual may try calling medical coding organizations and ask if the positions they are hiring for have the ability to become remote.

All members by 2010 must be certified by one of several credentialing organizations many of which include passing a competency test.

As members of the Association, all members adhere to a Standard of Practice and a Code of Ethics. When you begin to look for a care manager, be sure to inquire if they are a full member of the National Association and think twice about working with someone who is not. Another important question is in regards to availability. Is the care manager only available M-F and from 8-5? If so, what would happen in an emergency after hours? It is important to ask about charges. How are charges billed, hourly, a set rate, is there a charge for traveling, is there a charge for the initial meeting? And finally another important question to ask is whether or not the care manager receives any referral bonuses (kick-backs) when referring to services. This is a crucial question. If a care manager receives bonuses for referring to certain services, whose interest are they looking out for, your parents or their own? Some referral agencies only refer to those services that pay a fee to them. This means you are not getting all of the choices - only those who have paid to be on a list (keep in mind not always, but often, the services with the best reputations don't need to rely on referrals that they have to pay for). When picking a care manager be sure it someone you feel comfortable working with.

If you take just a little time to do your homework about choosing a GCM you will find the benefits to be well worth it. Rather than trying to figure out the entire system on your own, take that time and effort to find a GCM. You won't regret it!

The first thing you should do is research on adoption. The pros and cons, the different kinds, the time involved and what kind of research will be done on you. The job of caregiver, though usually unpaid, is one of the most complex and challenging roles anyone will undertake. There is no how-to manual that can adequately coach people on what it takes to keep a loved one safe and well once he or she has serious medical problems.

Whether you have a loved one living with you full-time or you manage his/her care from 3,000 miles away, chances are you will be called at some point to act as that person's advocate. So, along with the responsibilities that many of the more than 35 million caregivers in this country face on a daily basis, such as feeding, cooking, bathing or overseeing hired help, you will also need to be a voice for that person if he/she isn't able to ask crucial questions or find important information.

Physically or mentally impaired older people need advocacy in many areas. Some examples include dealing with housing or transportation offices, the Social Security Administration, and sorting through Medicare and Medicaid. Many seniors are entitled to benefits they are unaware of and need someone to help them find information on vital programs. Some elders are eligible for reduced or frozen rent rates, reduction of transportation costs and/or specialized transportation to help them remain independent. Patients and caregivers can cut down on the likelihood of injury by creating a simple medical record that is brought to all doctor's appointments and hospital admissions.

In addition to harm done by medications, what about all of the interventions that are offered to older patients or those with serious illness? We've all heard stories about someone who has had the wrong leg amputated or the wrong kidney removed, and are horrified that such things can still happen despite the high level of care available in this country. These are obvious cases in which people received the wrong care-care that only does them harm. In addition, another study showed a 13 percent increase in the number of procedures done on Medicare patients during their final hospital admission that did not result in a corresponding increase in life expectancy. We have more technology to use at the end of life, but it isn't always the right choice.

Because of this, it is critical that older persons and those with chronic serious illness (and their advocates) become well informed when treatments and procedures are being offered. Some important questions to ask are:

What will be the added benefit of this?

What are the possible side effects?

Will it affect my ability to function independently?

What are the possibilities if I choose not to do this?
