The practice of traveling to another country to receive some form of medical care is becoming more and more popular. This travel is often referred to as "Medical Tourism" or "Health Tourism." For some, the cost of a treatment in their country of origin is beyond their reach. Their insurance may not cover the procedure and by traveling to a country where the procedures are less expensive, they are able to get the treatment they need at a price they can afford.

There may also be long waiting lists in place for some procedures. Some countries that provide their citizens with public healthcare cannot provide non-urgent treatments in a timely manner and make people wait for months or even years. They would rather not waste time waiting in line when, in the same space of time, they could be well on their way to recovery in another country.

There is another kind of travel that is referred to as "Reproductive Tourism" that involves in-vitro fertilization or some other kind of assisted reproductive treatments. Again, many of these people travel for experimental procedures or for the anonymity of being in a foreign country.

Then there are also the travelers in need of various forms of mental hospitalization that would prefer to do this in a location that promotes relaxation and is out of the public eye.

You will definitely need to have a web presence with lists of travel packages to give people an idea of what they can expect. Make sure to refer them to organizations that provide information on foreign accreditations also, so that they will be sure to get the best of care. The choice of doctor and procedure will be entirely theirs. Your job is to help them get there and back and to make the trip as easy as possible.

One of the first decisions a business must make after deciding to incorporate involves selecting the proper state of incorporation. A corporation is not required to incorporate in the state of its operations. However, often the best decision may be to incorporate in your home state. (1) a dollars and cents analysis comparing the costs of incorporating in the state of operation versus qualifying to do business as a foreign (non-resident) corporation in the state under consideration and

(2) determining the advantages and disadvantages of each state's corporate laws and tax structure. If the corporation is a closely held corporation (not publicly traded) and does business primarily within a single state, local incorporation is often preferable. Designate Board of Directors

The designation of Board of Directors is legally required if a corporation is formed. The Board of Directors are essentially necessary for the management of the corporation. Their responsibilities include establishing all business policies and approving major contracts and undertakings. In addition, the Board may also elect the President. Ordinary business practices, however, are carried out by the Officers and employees of the corporation under the directives and supervision of the Directors.

Designate a Registered Agent

Almost all jurisdictions require that the corporation designate a registered agent for service of process. However, in most cases, anyone who has a street address (no P.O. Boxes) within the state of incorporation may act as a registered agent for the corporation. The main purpose of the registered agent requirement is to provide potential claimants against your corporation with a live person, whose whereabouts are available in public records, who may accept service of process on behalf of the corporation.

File Articles of Incorporation

Although state laws differ in their filing requirements of sole proprietorships, corporations must file "Articles of Incorporation" - the main filing document which begins the corporation's existence. Some state laws, such as Illinois, require a sole proprietorship to file an Assumed Name form with the County Clerk.

Hold First Board of Directors Meeting

After incorporation, the first Board of Directors meeting will resolve the following matters: adopt bylaws; issue stock; adopt a corporate seal; and designate a banking institution to serve the corporation's banking needs. Many states allow the Board of Directors to be comprised of one person.

Determine Accounting Methods

Each new business must also use a consistent accounting method, which is a set of rules for determining when to report income and expenses. The most commonly used accounting methods are the cash method and an accrual method. Under the cash method, you generally report income in the tax year you receive it and deduct expenses in the tax year you pay them. Under an accrual method, you generally report income in the tax year you earn it. Regardless of when payment is received, expenses are deducted in the tax year you incur them.

Obtain an Employer Identification Number (EIN)

To obtain an employer identification number (also known as a tax identification number) the IRS Form SS-4, available on the IRS web site at rs.gov, must be completed. The completed form can and be mailed or faxed to the IRS office indicated on the instructions to Form SS-4. There is no fee. If the EIN is needed quickly, the IRS can assign a tax identification number by calling at (866) 816-2065. The IRS requires Form SS-4 to be completed and present in order to verbally complete the process. The following questions provided by IRS will determine if your company needs to obtain a tax identification number. You will need an EIN if you answer "Yes" to any of the following questions:

(1) Do you operate your business as a corporation or a partnership?

(2) Do you file any of these tax returns: Employment, Excise, or Alcohol, Tobacco and Firearms?

(3) Do you withhold taxes on income, other than wages, paid to a nonresident alien?

(4) Do you have a Keogh plan?

(5) Are you involved with any of the following types of organizations? Trusts, except certain grantor-owned revocable trusts, IRA's, Exempt Organizations, Business Income Tax Returns, Estates, Real estate mortgage investment conduits, Non-profit organizations, Farmer's cooperatives and Plan administrators.According to the IRS website, individuals doing business as sole proprietors will not need an EIN, unless they decide to hire employees. All income for sole proprietors without employees can be reported using the owners Social Security number.

Register Business/Obtain License

Depending on state laws your business may be required to register with the Department of Revenue or obtain a retail license to sell/service to the public. Most states have sales tax laws designed to turn retail businesses into tax collectors. Call or go to your states Department of Revenue office and request the form for business registration to obtain a retail license. A small fee will generally be required and in some states you must renew the license each year. Once your permit is mailed to you, you will be given detailed instructions on how to file the sales tax forms each month.

Get a Business License

Many states and localities require business to obtain a business licenses or permits no matter what type of entity it is. The license can usually be obtained in person by going to the city or village hall business licensing office. Assisted Living Insider Secrets From a Veteran Administrator

Yesterday, an old friend contacted me, asking about placing his mother-in-law in an assisted living. However, after answering him in a series of emails, I realized that other people facing the same decision might be interested in the same information. Here's the interesting thing I discovered about myself in the process - my tone with him was different than if I were going to write for my blog or for an Ezine article. As a result, so were the tips that I would normally give - they were not very 'politically correct'. But thinking about it, I would not change a word.

In many ways, it was much better because of my mind set. I was focused on giving my friend advice rather than writing a "well written" article. So except for a few deleted lines that would identify my friend, here is some good advice about picking an assisted living:

Need your input - soon - We are looking at places (assisted living) for Mary's mom - she has continuing health issues primarily heart and respiratory. Would you look at the attachments and let me know your thoughts on these two places? Thanks so much, John

Hey John, I will look at the attachments closer later, but let me first highlight some important stuff to consider: First of all, if there is a non-refundable "community fee" of any kind, it is negotiable.

What is going to be more important than the room layout and the view and the transportation and all the other B.S., is how much care is going to cost. How do they charge for extra care? By levels of care? or by minutes? And when you find out - figure out what your maximum budget is, and what the most she will have to pay if she gets ALL their services.

Next, ask the facility about what would cause her to HAVE to move out? Ask them if they have a hospice waiver. (they are in California, so their rules are exactly like mine and I can tell you, if they do not have a hospice waiver, then when she gets towards the end, they will kick her out to a nursing home).

Ask them about house doctors (do they have a doctor that comes to the facility to see clients), and are home health happinesslifetime.com care agencies allowed in the building? and if so, do they require that you use theirs or if you can pick your own. I will explain why this important later.

Ask about wheelchairs. Are they allowed? What about motorized ones? Does mom have a dog or cat? Many place will let you bring it...for a fee. It too is negotiable.

The important thing to understand is how they charge between the room (and basic services) and care (optional services). They are much more likely to negotiate the room but they rarely negotiate care services. The best case scenario for you is to get them to lock in the rate for two years (or more). That is where they will try to screw you later, because they can raise their rent as much as they like. So make sure you trust the place. Anyway, think about this stuff and I will get back to you after I look at the marketing materials you sent me.

For a few hours after they finish, we play with our exotic parrots, the kids ride four-wheelers in the country around our campground, and I often spend the quiet time in the afternoon reading, researching, or watching a movie. At night... that's when I set the world on fire at my "home office."

Being able to schedule my work around my "peak times" and "bad mornings" is my favorite thing about working at home, but other parts of it are very important to me, too.

One of the best things is working with a laptop. I can take it anywhere - and I do. We're at our winter home right now, on beautiful Lake Bruin in Louisiana, and when the weather is nice, I just take the laptop outside, wi-fi button turned on, and I work on my creative ideas while I watch the clouds drift over the lake. The sound of wind chimes is soothing in the background, and those are the times when I think, "This is the life!"

Other times, the phone rings, it's someone with a big ad agency that I am dying to get together with, and the dog starts barking, the parrots begin to riot, and the kids come crashing into the room arguing... and those are times when I think, "I wish I were at the office!" But truth be told... I'll never go back. Even with the challenges of working from home, it's well worth it.

There are just a few things I would strongly recommend to anyone just getting started with a home office:

If at all possible, a separate room or area of the house that can be a "quiet place" for your phone calls
A combo scanner/fax/copier to take care of all of your office communications
A good, fast internet connection
A comfortable, truly ergonomic computer chair
A desk that is comfortable and has storage space, made for computer use
I can't emphasize enough - be good to your body when you work at home! Take care when selecting your chair, and don't forget about comfort. 2) WMD-TERRORISM Renegotiating to amend the NPT (with the most critical determinant being the negotiation with China):

a) For non proliferation of enrichment and reprocessing facilities. Did you know that the NPT currently allows enrichment and reprocessing facilities in any nation so long as they claim that these facilities are not for the purpose of nuclear weapons? And such facilities exist in Germany, Japan, Netherlands and Brazil among others? Once one has these facilities they are less than six months away from being able to have nuclear weapons. So that saying "we don't like you nor trust you" is the essence of the argument with Iran; not a solid legal argument.

b) for an IAEA bank being a reliable source (not the only source) for the supply of fuel for nuclear energy plants; and for the increased oversight, control, and disposition of all fissile materials by IAEA and

c) For measured verifiable WMD and nuclear warhead reductions by each nuclear weapons state,

Restructuring the UN, as described in the next Chapter, will help a lot; Longer term a verifiable proportionate military expenditures reduction, Treaty is necessary. There are only 20 or so uranium enrichment and plutonium reprocessing facilities globally. Fissile materials are quite limited, and easily controllable down to the gram; not doing so criminal insanity. To do so requires a renegotiation of the NPT.

3) DEFENDING DEMOCRACIES: Establish an economic alliance to include all democratic nations, including (in sequence), North America, EU, Japan, India, Brazil, Russia, South Africa and Australia is the most critical defense initiative for democracies. Expanding free trade within that alliance should more than offset trade reductions with dictatorial nations such as China and those within OPEC. This will be a result of appropriate import tariffs for Chinese and OPEC products.

Not recognizing that the standard by which free people should evaluate their actions is "what is best for the democratizing nations" is a major lapse in "common sense" among free humans. In terms of sequence, the increased political, military and trade alliance among nations that are democratic must be the first item because it is a prerequisite to all others.

There is a lot of chatter about the Georgia-Russia conflict and the implications for the West-Russia relations. Establishing a carbon tax (for things such as cars and SUV's with less than 30 m/hr mileage and private jets); for the "demand" side and cap and trade for national objectives and for the "suply" side (which domestically is best implemented by limiting it to utilities, refineries and iron and steel industry,) are the most critical strategic actions required domestically on this issue. The specifics of that renegotiation are discussed in the next Chapter however they are not achievable until items 1 and 2 above are achieved.

5) DEFENSE: MISSILE DEFENSE: Continued deployment of the anti-ballistic missile program, not in Eastern Europe, that does not need it and antagonizes Russia unnecessarily, but as near to the claimed potential threats as possible such as in Cyprus or Afghanistan is critical. This is central to moving Russia from aligning with fascist China. Fascist China is without question the major strategic threat of the free world.

ANTI TERRORISM: Defense against terrorism is not war because the terrorist are hiding; you can't have war without knowing where the enemy is; it includes battles in which significant military forces maybe needed and must be quickly accessible, but it is not a war. It is not a police action because it is too big and within foreign, not too friendly, nations and laws, for police action; even though it should include police action. The primary instrument for anti-terrorism is indeed offense rather than defense. It is by a covert, with strong intelligence to clearly identify the enemies, and thus probably through the CIA, (with increased Congressional oversight), commando force, (coming from current special military forces), in civilian attire, (connected to an expanded NATO Global anti-terrorist intelligence agency as well) of probably no more than 20,000 operating primarily in Pakistan, secondarily in Afghanistan and Iraq; and also in Saudi Arabia, Egypt and Iran.

Conflict against international terrorism is profoundly different than the wars that the military was and is designed to fight. The primary difference is that the combatants are dressed as civilians in civil settings. In winning modern wars the "moral" high ground and the "story" being disseminated to the public is critical. Having the military or police, dressed as such, operating in civil theaters, while a terrorist sometimes shows up, is by definition a losing "moral" impression to the public, becomes a police state, and a losing proposition. Conflict with international terrorism unfortunately, requires adjustment of our defensive combatants to be in offense, in the nations where terrorists operate, but mostly in civil attire and form. Increased presence in Afghanistan is necessary; the Taliban are now operating mainly out of Pakistan.

For as long as there are not at least 3000 US or NATO covert forces in Pakistan; preferably with the private approval of the Pakistani government, in exchange for the approx. $1.5 billion assistance they get for fighting terrorism; chasing Bin Laden primarily and other terrorist groups and the Taliban secondarily, nothing is being done against the center of terrorism.

It is expected that the governments of these nations will publicly refuse and deny the presence of US forces within their territory as part of their national sovereignty. Covert forces are called so because they are unacceptable politically or diplomatically as overt, yet are a necessary structural adaptation to the strategies of the terrorists. As a result regarding these covert operations, even though the public may not know for many years and thus is unable to judge whether anything substantive is being against terrorism, unless the covert forces as above are operating in Pakistan, nothing substantive is being done against the center of terrorism.

Strategic re-positioning of missile defense stations and the restructuring of how terrorism is to be fought, without affecting defenses against major wars, is the most significant strategic US military defense initiative.

6) DEFENDING US DEMOCRACY: CAMPAIGN FINANCE REFORM: Changing the laws on campaign finance to have all federal Senatorial, House and Presidential elections to be publicly financed, mostly through free ad time for Candidates, in the licensee's of public air waves, is the most significant action to restore and preserve US democracy.

In terms of sequence this is the first domestic agenda item that should be implemented. The current campaign finance reform (that was the major "accomplishment" of Sen. McCain's last Presidential Campaign) has left enormous amount of private money with self-only interest in the elections. The result is that the new definition of the best way to screw up any idea is to give it to the US Congress. Other than the example of Campaign Finance reform that came out as screwed up as it went in, is the current "global climate change bill" in the Senate. I have been advocating cap and trade for a long time until I saw this cap and trade bill, (about 500 pages of screwing everyone and their mama with bureaucracy.) It is truly disheartening. Campaign finance reform that takes all the money out of politics is a prerequisite to everything else because without it Congress will take each and all ideas and screw them to the point that they might as well never been offered.

7) WEALTH DISTRIBUTION: A one time re-distribution of some wealth (about one and a half-trillion dollars) through a small (average of about 15%) one-time tax, on those with net wealth of over 10m net, directly to the most vulnerable 70 million, current US citizen children, (all of the lower and middle class) at about $25,000 per child, without the government keeping any of it, and without increasing but rather, because of the reduced need for governmental programs, significantly decreasing (by over $50 billion/ year, through cuts and attrition) ongoing government spending; is the most critical social action within the US and within other nations that have the political will.

This action will automatically solve a series of structural imbalances that are now manifested in different ways. For example, the above action will automatically solve the current housing crisis as over 110 million US citizens will have significantly higher equity to allow them to maintain their homes.

Without this action the US economy is extremely likely to get into an economic depression, because the governmental spending and increased income taxes by the Democrats will lead there; and the socio-economic problems and increased military spending caused by the Republicans will also lead into a depression.

8) TAX REFORM: Changing the tax code to reducing income taxes of only the middle and lower class and simplifying the tax code as shown, and adding an equivalent in revenue Value added tax (a point of sale tax of about 20%, for not-manufactured here goods, with the exception of those goods coming from democratic nations) is the most significant trade initiative for the US. Strategically, increasing free trade and the economic, political and military ties with democratic nations must be done synchronously while having a regularly shifting targeted tariffs, taxes and barriers to entry structure, responding to the political actions of dictatorships, for products from dictatorial nations; specifically with China and the OPEC nations in mind.
