In-home childcare can take on many names, e.g., au pair, nanny, babysitter, mother's helper. Parents can find in-home childcare through placement agencies or by performing their own search and interview process. When a potential candidate is ready for the next steps, a telephone screening is recommended as well as personal background checks.

In this Internet age, an online search can be used to find any information on the prospective employee. After the initial screen, you'll want to schedule an initial interview. This first interview is an opportunity for both of you to become acquainted and ensure your attitudes toward child rearing are aligned. The children should not be around for this initial interview, so you may get to know each other without distractions.

Encourage the candidate to talk about previous experiences and ask why they enjoy working with children. A good interview should entail both talking and careful listening. Does the interviewee ask about your child's schedule, temperament, likes and dislikes? How much do they know about the developmental needs of children the same age as yours? Coming Home - 5 Things to Prepare For Your Parent's Homecoming From a Skilled Nursing Facility

Coming Home

Bringing a family member home after a stay in a skilled nursing facility (SNF) or rehabilitation center, or directly from the hospital, can be an overwhelming challenge emotionally and physically. If you are going to be a caregiver either in your own home or your parent's, to minimize emotional overload you need a plan. I'm going to give you one. First, let me share my story, and then I'll recommend what to do.

My Mother's coming home tomorrow at 2 p.m. "Home" I've learned over the last three years is wherever your loved ones are. I have been traveling from my "grown-up" home in Virginia to my "childhood" home in California, looking after my 86-year-old Mother (only child of only children). It's been a joy, but whenever I leave one place, I say "I'm going home." This last trip I decided both are home -- so I now refer to them as "the ocean" and "the farm."

My mother's coming home tomorrow at 2 p.m. is real for her. This has been her main residence or second home in California for over 30 years. I've been a nomad in it as I have come and gone to look over her care and be with her in a beautiful (we have been blessed) skilled nursing facility close by.

But it struck me one night as I lay in my nomad state watching TV that she was doing the same thing several miles away, so why shouldn't we be doing it together? She has been in the facility for three years. They had taken great care of her. When I left to go to the farm after her first hospitalization, I worked hard to get caregivers for her at home who would care for her in my absence. She wasn't really sick, just immobile and afraid to be touched. She graciously growled at every lovely lady who came in, and I again knew in that moment, if I left, she'd kick them all out, so it was back to the facility, which kindly took us in.

Fast forward three years and she's been well cared for. Her mind is still sharp. Yet, she had come to the point where all she wanted was to lie in bed, be left alone, not eat (except for the chocolate I brought ever day) and said repeatedly she was living too long. In our care plan discussion, the lovely hospice social services lady, Paula, told me to be prepared, that it was not unusual for women to just start shutting down until they pass away.

Then a funny thing happened on the way to "home." My mother rallied. Three days before the scheduled departure I get a call from wonderful, Kit, the Head of Nursing. "I've got the greatest news. Mom has been up in her wheelchair, sitting in the Activities Room, taking all three meals in the dining room" -- which she had always called the "old people's room" and refused to go into, preferring meals in her room. I'm told the staff in amazement is calling this the "Awakening of Mrs. Lee."

Her front room with the ocean view is all ready to go, new bed (not a hospital bed, you need a special bed~more later), a lovely sofa bed to sit in by day and hang out with her all night if necessary, all my "little girl" furniture of which I was most proud has been taken away to make room for this new era in our lives.

My mother's coming home tomorrow at 2 p.m. -- from now on that's where I'll be hanging with one-half of my family at "the ocean." What joy.

I want you to have the same experience -- with or without the ocean. Here's what you need to do:

1. If your loved one is in a SNF, hospital, or rehabilitation facility, sit down about a week before you are planning to bring your parent home, listen and ask lots of questions!

2. If your parent is on hospice, hospice will offer a hospital bed at no charge. The standard in the industry says they are unbearably uncomfortable. If finances are an issue, you may have to take the bed. It is a blessing hospice even provides all that they do. If you are able to spend some money, I recommend checking into a nursing home bed distributor. Some of those beds are better than others. I worked directly with my "team" at the nursing facility (they have connections, you just have to ask and push) to arrange for a top-of-the-line bed to rent for $150 per month. The bed can also be purchased. The IMPORTANCE of this type of bed is that it moves the body in all different positions, crucial to prevent bedsores, and allows the bed to go all the way to the floor if your parent has a tendency to fall. Hospital beds don't go to the floor.

3. Hospice will also provide an eating table that goes over the bed for meals and holds drinks during the day, a wheelchair, oxygen, and a hoyer lift, which takes us to #4.

4. There are different sizes and kinds of hoyer lifts. You will want to talk to hospice about what they can provide, or check on your own -- Google "hoyer lifts" -- to find the right one for you. They will aid you in lifting Mom or Dad out of the bed and putting them in an armchair or wheel chair. You need practice, so ask the facility to show you how it works and practice with your Mom or Dad before they come home.

5. Finally, but equally important, finding caregivers to support YOU! You cannot take care of your parent 24 hours a day -- it is a physical and emotional impossibility. Watch out or the bed your Mom or Dad vacated might soon be yours! Reputable, trustworthy, bonded caregivers are the key. Ask your facility for whom they know to be the most trustworthy caregiving agencies. You will want to interview the caregivers and introduce them to Mom or Dad. If one doesn't work out, don't hesitate to ask for another. This is the most important step--you are entrusting your parent's life to another so that you can get out and have some recreation time, etc., AND you want someone in your house you can trust while you're gone. Put any valuables away. No matter how great we believe someone to be, we never truly, truly know them, especially in this setting.

You can do it. Loving your parent through this difficult time is a challenge, but will give you those special moments you will treasure forever. Corny to say, but love is a powerful thing, and serving your Mom or Dad at this time in your life is healing to them and a gift to you.

Elder Care Business in 2009 - Opening and Marketing Your Dream

I love the start of a new year. It brings to mind that the possibilities are endless, and for many of us, it's a great time to start something new or different in our lives.

I'd be a rich person if I had a nickel for everyone out there who has decided to open a new in-home care agency, open a professional geriatric care management practice, or start some sort of elder care business. (I get a call or email about this at least 3 times per week consistently!)

What I know for sure is that there is plenty of business to go around. As we live longer, as our parents and grandparents live longer, we create an unlimited opportunity and need for senior related services.

The key is to create an appropriate and realistic business and marketing plan that generates clients and results. The key to the senior market, is not surprisingly, their adult children.

If you have been in the senior market for any amount of time, you know that families often contact senior service providers when they are in crisis due to an acute illness or other event that signals the need for services and support.

So whether you are just starting a new senior/elder care business, or already own one- think about marketing to more than just seniors. Your target audience may be a little younger than you imagine!

Consider some of the other things we have talked about over the last few months:

o Issues related to senior living options, senior care options, and financial issues that affect seniors require education, so educate your prospects.

o Advertising with vanity ads and the "same old thing" is "OUT", and offering information in the form of a free report, CD, seminar, or consultation is "IN".

o Directing messages to baby-boomers who HAVE aging parents is key. Finding them is not as easy as buying a "list".

o Understanding financial options for seniors is a MUST for all senior service providers. It's impossible to offer good advice to a family if you aren't up on the latest programs and strategies.

o Networking is possibly the most effective and least costly form of advertising and marketing. You should attend only the networking events that make sense for your business. Pick 2-3 key groups and stick with them. (But try as many as you can, it never hurts to check out a new group!)

o Finally, when you are budgeting for your marketing, budget for consistency and frequency, not the big bang. This market does not work like that.

o The reality is that you must touch a consumer 6-10 times- either through print, radio, or TV, in order to get a response. o Running a radio ad 4 times and pulling the ad because you didn't get a phone call is not the way to execute a marketing strategy.

o This is a sensitive subject for many (long-term care needs), so in short order you must articulate your message, interrupt your target (ie get their attention), engage them, educate them, and then make an offer.

o Offering something for free- your time, your printed report, your audio CD, etc, will get the most response. Some people like to remain anonymous...so let them, but engage them.

Finally, this is a very good year to live your dream, be an entrepreneur, and serve the greatest generation of all time. Take a realistic, slow but steady and consistent course of action, and you will succeed! We need more senior service providers who really know their stuff. Women and Abundance

There is a coming crisis regarding womens issues on aging and their financial future. "Nations of the world face the challenges of helping aging women attain economic security and promoting the productive potential of woman as they grow old." An International Report Prepared for the United Nation Second World Assembly on Aging.

The purpose of this article is to explore some of the economic issues facing women as they age. Is there a solution to these issues?

What are the key points of concern for women of the baby boomer generation?


In 2004, the average income for a single woman age 65 and older was $9,430 a year. In the United States, approximately 70% of older households have annual incomes below $35,000. Almost 30% of those households have incomes between $10,000 and $20,000. The highest rates of poverty are found among older single women, a category that includes widows and divorced women as well as those who never married.



In 2003, 3.4 million Americans age 65 and over live below the poverty level, 71% were women. About 3.6 million elderly persons (10.1%) were below the poverty level in 2005. This change in the poverty rate was not statistically different from the poverty rate in 2004. Another 2.3 million or 6.6% of the elderly were classified as "near-poor" (income between the poverty level and 125% of this level). Older men were much more likely to be married than older women, 72% of men, 42% of women in 2005. 43% of all older women in 2004 were widows.



For one out of four of these women, social security was their only source of income. As primary caregivers of the house, they contribute less to their social security, and are generally paid less when they do work. Many of the widest pay gaps occur in the best-paid jobs with the most highly educated workers. Among physicians and surgeons, the highest-paid occupation for men and women, women earned 63% of what men did. This was the same ratio for female and male CEOs. Female lawyers earned 74% of what male lawyers earned. 101 Facts on the Status of Workingwomen, Published July 2005.



Half of older women age 75 and older live alone. By 2020, this total will be 13.3 million and 45% of these women living alone are poor or near poor. (same article as above). The number of childless women has grown to an alarming rate - there will be no one to care for these women in their old age. The average American life expectancy in 2003 was 77.6 years. (80.1 women and 74.8 men) Women live longer than men by an average of 5.3 years. This means that about 85% of women die alone - either unmarried, divorced, or widowed.



New vulnerabilities concerning standard of living in retirement: Declining stock prices after the crash of the high-tech sector in 2000 pushed many pension plans into deficit status and undermined the value of individual retirement savings.

For many women, facing the "golden" years is going to be difficult. Lack of planning and changing economic status will have them in a financial bind.

What are the Possibilities?

The number of women-owned businesses in this country grew at twice the rate of all firms between 1997 and 2002, jumping 14 percent to 6.2 million, according to the Center for Women's Business Research. Women-owned businesses account for 28 percent of all privately-owned businesses, and employ 9.2 million people. They contribute $2.38 trillion in revenue to the U.S. economy, according to the Small-Business Association. Nearly half of all small businesses are owned by women. (President Bush Discusses Jobs & Trade at Women's Entrepreneurship Forum, Cleveland Convention Center.) Another study found that the number of female entrepreneurs is growing at 3.3% annually which is greater than the number of men who are becoming entrepreneurs.

Aging baby boomers will lead a surge in start-ups that will accelerate the economys recovery. The number of self-employed 55 and older entrepreneurs has increased 18% from 1994 - December, 2003. These senior entrepreneurs now represent 28.7% of all self employed workers, the highest percentage among all age groups, according to the Bureau of Labor Statistics. These "seniorpreneurs" will be major employers of other aging boomers like themselves, ensuring that the unemployment rate among older workers will continue to remain the lowest of all age groups.

To a lot of boomers, retirement means continuing to work in their own way and reinventing themselves.

Is there a solution?

Turn the Tables on Debt Collection Agencies

When you first get into a debt problem situation with unsecured debt, you may have picked up the advice that you should notify all your creditors about your problems. I'm sure that this is a good idea, but don't expect it to change the way that your creditors deal with you. In most cases they will pass your details to their own debt collection department or even an external debt collection agency that will pester you endlessly over the phone and by letter. It doesn't matter if you told them 2 days ago that you could not pay, they will still phone again just to make sure that you can't pay now (and to keep the pressure on).

How You Can Turn the Tables on Debt Collection Agencies

Most people don't realize that you do not have any obligation to speak to your creditors about your debt problem. With the exception of secured debt such as your mortgage, speaking to them doesn't usually change the outcome. Creditors will follow their own debt collection procedures more or less whatever you do. Once you have notified them you have a problem your time is much better spent considering how you are going to resolve the situation (e.g. Debt Management Plan, IVA, Bankruptcy).

If you dread picking up the phone then there are things that you can do.

1. Use your answering service! Most phone companies provide a free (or very cheap) message-taking service. Get into the habit of not picking up the phone and let whoever is calling leave a message. When you listen back then delete the message as soon as you work out that it is from a debt collection agency.

2. Change your phone line (through your service provider) so that it doesn't accept callers which withhold their number. Most debt collection calls will originate from large call centers that have the "number withhold" set. Using this tactic will cut out a surprising number of calls.

3. Change you number to an ex-directory one. Make sure if you do this that your own number is set to "withhold" otherwise people will gradually get to know it. Do not put your number on any official forms of any kind.

If opening the morning post is a distressing experience for you then get into the habit of scanning the top part of any letters so that you can work out whether it is from a debt collection agency. If it is, then throw it away without reading it! Most of what it contains will probably be unsubstantiated threats and lies anyway so why depress yourself. Do keep any eye out for any court documents, as these do need urgent attention.

If someone does call at your house to try an collect the debt (and for most debts this is extremely unlikely) then get them to establish exactly who they are. If they are a debt collection agent you are under no obligation to speak to them. If they refuse to leave your property the they are actually trespassing! If they are a bailiff you have to be a bit more careful. For most personal debt they do not have the right to enter your property. In fact, it is vital that you do not let them into your own property (or leave any doors unlocked or windows open that they could get through). They cannot push past you when you are standing at the door but if you let them in or they gain access through an unlocked door or open window then they can start removing your goods. It is worth stating again - NEVER LET A BAILIFF INTO YOUR HOME FOR ANY REASON. They will sometimes try and say that they need to use your phone or some other trick to gain access. Don't fall for it.

The idea of turning the tables on debt collection agencies is not to enable you to ignore your debt problem. If you ignore it for long enough it will get to the stage where drastic legal measures are taken. You should use the breathing space given by these techniques to consider what the long-term solution to your debt problems is going to be, and get some good free advice.

o Veterans can use the veterans Administration for their LTC needs. The Veterans Administration does assist many veterans but the waiting period for assistance can be years and still doesn't guarantee a bed.

o My family will care for me is also an old concept not practical in today's society. In many households both spouses need to work to support the family and not often are their assets in place to care for a parent.

Are there options to help me decide which policy to buy? The answer is yes, numerous sources exist and many agencies will help. A solid tip is to ask for the insurance company's financial rating. The rating service will evaluate the company as to their claims paying ability and their ability to continue into the future. Many times the lowest premium is not the best, always ask for several options form your agent and never feel rushed into a decision. Take your time and make a logical choice based on your personal situation.

Minding Our Elders: Dad - Just Give Me the Keys!

Alice came in saying she’d gotten lost. My mother-in-law had gone to the grocery store – the same one she’d shopped at for thirty years. It was four blocks away. I was staying with my father-in-law, because he couldn’t be alone. A couple of hours passed and I was worried. Finally, I heard her footsteps in the hall.

She looked shaken. Confused. I asked if she was okay, and she said she was, but there was a hitch in her voice. Then she said she had gotten lost coming home. That was the last time she drove.

With my own mother, it was more dramatic. I went to her parking garage to get her car so I could get it filled with gas and get it washed. I noticed the side was caved in, and the yellow marker she parked by had blue paint on it. The same paint as her car. When I went back to her apartment and carefully mentioned the car, she looked totally confused. She didn’t remember anything. It must have been someone else. We got the car fixed, but it happened one again in a week. She finally came out of denial and realized she shouldn’t drive anymore.

Giving up driving is one of the toughest things anyone can do. It’s not just the driving itself, but the sense of independence it gives most people. When you can no longer choose to hop in your car (or even toddle to it behind your walker) and go somewhere – anywhere - you know you have lost something. My experience is that it’s tougher on men of my parent’s generation, than women. The men tended to identify more with their vehicles and also their independence. My dad gave in gracefully, when his eyes failed, and my father-in-law never really quit – he was just slowly losing the energy to even get to the car, so it happened naturally. But my uncle gave in as gracefully and a wounded elephant. Only a massive stroke stopped him completely.

What about the senior who won’t give up the keys? Who insists he or she can drive when you know it’s not safe for them, or for anyone else? There are a few options to try before “losing” the keys down the sewer grate.

If you are lucky, and your elder has had a particular doctor for an extended time, the doctor’s advice may be enough to convince them. But not always.

I’ve found three places with helpful advice. One is The Hartford. The site, [thehartford.com], offers brochures that have great tips on helping seniors know when to stop driving and more tips to help caregivers convince those who shouldn’t drive that they need to give up the keys. Your area Agencies on Aging site also has resources on older driver safety at n4a.org n4a.org. AAA puts out an interactive computer program where a senior (or anyone) can test their own driving skills, in private though a partner is needed. The program is titled “Roadwise Review,” and can be ordered on-line or you could call your AAA office. The company also has a series of booklets: “Straight Talk for Mature Drivers.” The series includes, among others, “Meeting the Challenge,” which talks about ways to compensate for the effects of aging on driving and “A Flexibility Fitness Training Package” suggesting exercises that can help driving performance. AAA maintains that they want to help seniors drive as long as it’s safe. Sometimes, this approach can help extend driving time. If not, it may help convince the senior that it’s time to quit.

Finally, many hospitals and clinics have programs built in, where they will do a physical and psychological exam, and then have the senior take a trained staff member for a drive. They set up the driving to be as close to the experience the senior would have when driving around his or her neighborhood or town. If the person is from a small town, they would be taken to a quiet neighborhood. If they drive in traffic, they would be asked to take the trainer into traffic. Check with your local hospital to see if they have such a program.

The bottom line is safety – for the senior and for others on the road. If you are having trouble convincing a senior to give up the keys, try some of these resources. Oh – and good luck.

Most people are retiring in better health than ever before and can look forward to decades of an active and fulfilling life. When you first retire, especially if you retire early, health care might be one of the last things on your mind. But as we continue to age, health issues inevitably begin to crop up, and eventually we may need some form of assistance.

Traditionally, seniors citizens who can no longer live independently have moved in with their children or other family members. This is still the case in much of the world; particularly in Asia, children feel that they have a firm responsibility to take care of their parents regardless of the sacrifice that might entail, reasoning that their parents gave them life and raised them from birth, creating an obligation to return the favor in whatever way possible.

In Western traditions, senior citizens more often feel that they don't want to burden their children or other family members; the children may be living far away, with busy lives and financial obligations toward their own children. Many seniors prefer living independently, even alone if one spouse has died, and communities have responded by establishing senior centers and various other facilities and services for senior citizens.

If a senior is no longer capable of living completely independently but does not yet need full nursing home care, various levels of home care are available, from full professional home health care provided by a registered nurse to life assistance services in the form of meal preparation, laundry and housekeeping, shopping and other errands, and companionship.

A senior's capabilities are broadly defined in the senior care industry by the term "Activities of Daily Living" (ADLs): bathing and grooming, dressing, going to the bathroom and controlling one's discharges, "transferring" (getting out of bed, or getting into or out of a chair), eating, and walking. These basic ADLs may be defined somewhat differently by different organizations. In addition, there are "Instrumental ADLs" (IADLs), which are not necessary for fundamental daily functioning but are generally necessary to allow independent living: doing housework, preparing meals, taking medication correctly, shopping, using the telephone or other technology, and managing money. Health care practitioners and case workers will determine how many ADLs and IADLs a senior is capable of performing in assessing home care needs.

The simplest home care services involve a person coming to the senior's home on a regular basis - often daily, from a few hours to the full day - to help with ADLs and IADLs as necessary and provide companionship. Sometimes, a housekeeper is all that's necessary, but if the senior has any potential medical problems, the home care provider should at least be able to recognize symptoms and know how to response in case of emergency. Certainly, experience in working with a senior would be a minimum requirement for any potential candidate.

There are many agencies that provide home care services, and the individual care provider should already be screened and experienced. If one person doesn't work out, simply call the agency and request someone else. Alternately, you can find a private direct-hire home care provider. However, be sure to get references, and follow up on the references. With no agency vouching for the individual's capabilities or experience, you need to make sure the person is capable.
